Registration Process at WMMC

Patient Registration

Registering at WMMC helps us keep your healthcare records accurate and up-to-date. This includes your personal information, contact details, and insurance.

As part of registration, you will be asked to sign forms that explain your rights, how your medical information is used, and your consent for healthcare services.

 

Before Your Appointment

What should I bring?

  • Valid driver’s license
  • Insurance card(s)
  • Co-pay or deposit for your service

Hospital & Clinic Hours

Our main hospital entrances have standard hours:

  • Emergency Entrance: Open 24/7
  • Outpatient & Main Entrances: 6:00 a.m.–9:00 p.m.

Who is at the entrances?

Volunteers are available at each entrance to help you and other visitors find what you need.

When you register at WMMC, you will be asked to review and provide consent for the following:

  • Consent to Treatment: Gives general permission for medical treatment at WMMC and with our care partners.
  • Release of Information: Allows WMMC to share your health information in accordance with HIPAA for purposes of treatment, payment, or healthcare operations.
  • Assignment of Benefits / Medicare & Tricare Benefits: Authorizes WMMC to bill your insurance on your behalf and receive payment directly.
  • Agreement to Pay: Confirms that you understand you are responsible for payment of services and the expectations regarding billing and collections.
  • Personal Valuables: Acknowledges that WMMC is not responsible for loss or damage to your personal belongings.
  • Patient Bill of Rights: Confirms that a copy of the Patient Bill of Rights has been made available to you.
  • Consent to Telephone Calls, Email, or Text Messaging for Healthcare: Allows WMMC and our care partners to contact you using HIPAA-approved communication methods.
  • Complaints: Explains how you can file complaints or share concerns regarding the care or services you receive at WMMC.

 

Accessing Your Health Information

In addition to completing your registration forms, WMMC provides ways for you to access your health information:

  • Medical Records: For copies of your medical records, contact the Medical Records Department at (660) 262-7395.
  • Patient Portal: View lab and radiology results online through the Portal del paciente once final results are added to your record. Not all results are available; tests sent to outside labs or physicians may require a visit to Medical Records or your Primary Care Provider.
  • Portal Support: For login questions or technical help, contact Cerner Support at (877) 621-8014.
