Community Health Needs Assessment (CHNA)

Guiding our community health priorities:
The Community Health Needs Assessment (CHNA) helps WMMC identify and address unmet health needs in the communities we serve. Conducted every three years, the CHNA uses community input and data to guide health improvement efforts and community partnerships.
Our most recent CHNA was completed in March 2025.
