That’s why we offer the Connected Care Program, an innovative healthcare service designed to empower you in managing your chronic conditions from the convenience of your own home.
WMMC Connected Care is a service for patients diagnosed with any chronic conditions, including arthritis, diabetes, heart disease, and high blood pressure. Patients in the program can directly text their Chronic Care Navigator to ask questions about their health, request prescription refills, schedule appointments and more.

Our dedicated care team of Connected Care Navigators are here for you whenever you need them. No more waiting on hold or navigating through automated systems. With a simple text message, you can reach out to your personal healthcare assistant for immediate assistance.

Connected Care may have a small cost sharing monthly payment depending on your insurance, but it can help you avoid unnecessary hospital visits and save you hundreds of dollars for each prevented visit. Patients can opt-out of the program at any time. Contact your health insurance provider for more details regarding these costs.
