Social Services
An admission to the hospital can create anxiety for both the patient and the family. Our Medical Management Department of registered nurses and social workers are advocates that help patients understand their current hospital status and treatment plan. The Utilization Review Coordinator and Care Coordinators, as well as our Discharge Planners, can help guide the patient through a complex and often confusing health care delivery system.
Care Coordination
To help identify appropriate providers and facilities through the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner.
Advance Directives
This service includes a Living Will or Health Care Directive. If you do not have an Advance Directive and request information, our staff can provide and assist you with the completion of this form. In addition, notary services are provided through our department for free as well.
Discharge Planning
Our social workers may visit to complete a comprehensive assessment regarding possible discharge options, which may include but are not limited to: home health care, medical equipment, transportation, rehabilitation placement, hospice, nursing home placement, residential care placement, and referrals. In order to meet your discharge needs our social workers may refer you to different community, state or federal agencies as well.
Utilization Review
The process by which health insurance companies, other health providers, and hospitals determine whether health care diagnosis and treatments are medically necessary for the patient. The process is the evaluation of the medical necessity, appropriateness, and efficiency of the use of health care services, procedures and facilities under the provisions of the applicable health benefits plan. The status of the patient during the admission to the hospital is also decided with collaboration between the utilization review coordinator and the physician.
